POD Step 1 - Application 

Let’s get started. Welcome to the first step of the POD Program. The application is your opportunity to introduce yourself, share your goals, and take the first step toward building a future that aligns with your values and aspirations .

• You currently live in a household with below $63,000 per year (Based on a 2-person HH)

• You possess a growth mindset. 

  • • Growth mindset: definition, characteristics, and examples 

1.  Complete our Participant Application: Introduce yourself, share your goals, and tell us why you’re interested in the POD Program.

2. Schedule your Introductory Meeting (A link to book your meeting will appear after you submit your application.)

3. Complete the Enrollment Package (see: Forms) 

4. Submit Required Documents 

   • • Government- or state-issued photo ID 

     • Social Security card

     • Work Authorization card (if applicable) 

     • Latest school transcript (if applicable) 

5. Complete a Drug Test 

   • • You may take the test at a clinic of your choice. Be sure to email the results back to us. 

     • Prefer a free option? Visit M.D. Medical Clinics. Print and bring the attached Drug Test Authorization Form with you.
